This draft describes the new website’s current features. The clinic must confirm its privacy contact, retention periods, service-provider arrangements and privacy practices before publication.
Information you choose to provide
The general inquiry form collects your name, email, optional phone number, reason for contacting and message. Do not include detailed health information, insurance identifiers or other sensitive records.
How inquiries are used
Inquiries are stored in the website database and available to authorized staff so the clinic can respond. This form does not book appointments or subscribe you to marketing. If email notifications are configured, the notification contains a reference number rather than the message itself.
Booking and third parties
Online booking opens ClinicSense, which operates its own booking and intake service. Social links open third-party platforms. The optional map loads Google only when you choose to load it. Review each provider’s privacy information before sharing data.
Website analytics and staff access
The website is configured for Floot analytics using a session identifier held in memory, without persistent analytics storage. Staff sign-in uses an essential session cookie. No Meta Pixel or marketing subscription is active.
